RAFFLE TICKET ORDERS
The price for one (1) raffle ticket is $25.00, and for five (5) raffle tickets is $100.00, Any prize awarded with respect to any ticket will be awarded to the person named on the raffle ticket. Rand Eye Foundation is not responsible for lost, misdirected, late, non-delivered, or other failed efforts to submit a timely ticket order. All ticket orders must be received and fully paid prior to the drawing date of February 28, 2013 in order to be valid. The IRS does not consider the purchase of a raffle ticket to be a tax-deductible charitable contribution. Please consult a tax adviser for personal tax matters. All sales are final. ALL FEDERAL, STATE, LOCAL, AND MUNICIPAL LAWS AND REGULATIONS APPLY. VOID WHERE PROHIBITED.
ELIGIBILITY
Rand Eye Foundation’s is open only to legal residents of the United States who are at least 18 years or age.
WINNING THE PRIZE
Winner need not be present at the drawing; winner will be notified by phone and/or email. Rand Eye Foundation will make reasonable efforts to contact the persons named on the winning raffle tickets purchased in person or via online order form, using only the information on the winning raffle ticket. All prizes must be claimed by winner within seven (7) days from date of drawing. The person named on a winning raffle ticket order form agrees, as a condition to being eligible to receive any prize, to provide written information as is required by any applicable tax laws, including without limitation his or her photo ID as proof of identity in forms satisfactory to Rand Eye Foundation showing that the person claiming the prize is the same person who is named on the winning raffle ticket. Rand Eye Foundation shall be protected and may rely in its sole discretion any information on it provided to it by the person presenting a winning raffle ticket to confirm that the person is the person identified on the ticket.

LIMITATION OF LIABILITY/HOLD HARMLESS AGREEMENT
Each person who purchases a raffle ticket and/or whose name appears on a raffle ticket order form: (a) acknowledges and agrees (for himself/herself and for every successor in interest) that the terms stated in this instrument are also conditions of Rand Eye Foundation’s obligations hereunder, and that any obligation of Rand Eye Foundation is limited solely to its undertaking to provide a prize to the winner of the drawing as herein provided, subject to the stated terms and conditions, and that Rand Eye Foundation shall in no event be subject to any other obligation or liability, including but not limited to, any consequential, special or punitive damages of any kind; (b) agrees that Rand Eye Foundation, its directors, officers, employees, agents, representatives are not responsible for any injuries, losses or damages of any kind arising in connection with, or as a result of, this raffle and/or that person’s failure to win or redeem any prize, or that person’s award of, acceptance of, or use of, any prize; and (c) releases and agrees to indemnify, defend, protect, and hold harmless Rand Eye Foundation its officers, directors, employees, agents, and affiliated entities from and against all losses, damages, rights, claims and actions of any kind arising in connection with, or as a result of, this raffle and/or that person’s failure to win or redeem any prize; and (d) represents and warrants to Rand Eye Foundation that he/she has read and understands these Rules and agrees to be bound by these Rules and Rand Eye Foundation decisions, which will be final.
